Meet Rose Gichomo, BSN, RN, CCM
LightWay Care Management was founded by Rose Gichomo, BSN, RN, CCM, a Registered Nurse with more than 20 years of experience supporting older adults and families through healthcare and aging-related challenges. Her background includes long-term care, assisted living, memory care, home health, and care management.
Rose is also a Certified Case Manager and Aging Life Care Professional® with experience coordinating care, advocating for clients, and helping families navigate complex medical and care decisions.
“As a Registered Nurse, I know how overwhelming it can be for families and caregivers trying to figure out where to start, what resources are available and how to access the services and supports their loved one needs when unexpected events such as a fall, acute illness or cognitive decline impacts a client’s level of functioning. My goal is to partner with the client, their loved ones, caregivers and providers early enough and provide person-centered planning, guidance and holistic solutions that reduce stress, eliminate barriers and illuminate the client’s healthcare journey.”
